Codex code review stuck on "Running" on a GitHub PR, and a new "@codex review" gets 403

Codex "Code Review: Running" never finishes GitHub PR "Resource not accessible by integration"

1What happened

On a GitHub pull request, the Codex connector took an "@codex review" request. Its summary said "Code Review: Running" and never finished.

The required review status was never posted, so the PR could not merge.

Earlier the same week, the same kind of hang held a change for about 12 hours, with no alert.

What did not work, 1: closing and reopening the PR. The review did not restart.

What did not work, 2: posting a new "@codex review" from a GitHub App token. GitHub refused it: HTTP 403, "Resource not accessible by integration". Only a Codex-connected user account can ask for a review that way.

2What we did

  1. Pushed one empty commit to the PR branch. The new head commit gets a fresh automatic review.

    For builders
    git commit --allow-empty -m "re-run Codex review"
    git push

3How we know it worked

On Fri Oct 23, Codex reviewed the new head with no P0 or P1 findings, and the PR merged at 7:35 PM PT.

4What you can take from it

When an automatic reviewer hangs, give it a new commit to review instead of asking again. And alert on a review stuck past 30 minutes, so it never sits for hours.
